 1.  FCC&S Pre-Admission Application and Application Fee

For students who are seeking our degree programs, please download, print and answer all questions on the application.  If you are unable to download an application, please call our Admissions Office to have one sent by mail.  If a question does not apply to you, please mark the answer N/A.  Once completed, along with your Statement of Faith, (3) Letters of Recommendation, and your Official Transcript Requests sent to previously attended colleges, submit the application to the address and department listed below with a non-refundable fee of $30 for processing.

 2.  Academic Transcripts

All official transcripts must be received in sealed envelopes from each school.  Use the Official Transcript Request Form to request transcripts from all previous schools.

High School Transcripts: If you have not attended a higher education institution prior to applying to Foothills, you will need to download and complete a Transcript Request Form and submit it to the high school from which you graduated from or expect to graduate from. We request the most current transcript be sent now, and then a final transcript to be sent after your completed high school graduation.

Home-school Applicants: To verify the completion of secondary school education program, submit documentation from your local school board, public school superintendent or a signed affidavit by a parent or guardian.  Include a copy of your home-education diploma, along with a concise transcript or record of your the last three years of home education. Please make sure the graduation date is included on the home education records.

GED: If you have received a GED, contact the state Department of Education where completed the test and request an official transcript of your scores to be sent our Admissions Office.

College: If you have previously attended another college, please request an official transcript from all attended colleges to be sent to our Admissions Office.  If a transcript request form is needed, one is included in the application packet.  All official transcripts must be received by our Admissions Office in order to transfer credits from other institutions.

3. Statement of Faith

Fill out the essay form provided in your own words.  This essay may also be submitted in typed form. Submit this with your application and fee to the Admissions Office.

 4.  (3) Letters of Recommendation

Two Personal Recommendations and one Pastoral Recommendation

Fill out the top portion of these forms before they are given to your references.  These recommendations are to be completed by someone who has known you for at least one year and cannot be completed by family members.  Enclose the form and an envelope addressed to the college    so that each form can be sent directly from your reference.                               

* Once applicants have completed the required FCC&S Application and Statement of Faith, submit to the following address, along with the non-refundable $30 fee.  Once received, a transcript review and course recommendations are possible.
